One of the largest strategic issues facing health and social care is how the council and health organisations manage to support the growing number of older people, people with disabilities and those with long term conditions.
Factors such as increasing pressures on budgets, a diminishing younger population, dispersed families, pressures on informal family carers, greater complexity of care needs and more people with disabilities living longer all create pressures upon health and social care.
The aim of the Better Care Fund programme is to change the way in which people access services, both in and out of hospital, with a reduction in acute hospital-based activity and an increased use of home-based and community support. These changes should not only deliver more joined up care but also improve people’s experiences and health outcomes.
