Carole Johnson - Community Development Coordinator including Wheels to Work

Carole Johnson - Community Development Coordinator including Wheels to Work

Carole has previously worked on The New Horizons project which was funded through the Government Office of the Civil Society Transforming Local Infrastructure programme. The project aimed to improve access to high quality redeveloped support, training and volunteer brokerage, strengthen links between the sectors and promote corporate social responsibility activities with private and public sectors by securing longer term agreements to improve the sustainability of voluntary and community groups.

Carole also works on the Social Prescribing Project in East Yorkshire. This programme works with local GP practices to identify and support patients who are suffering isolation and loneliness but which does not fit with a medical intervention.

Carole continues to support rural communities in her role as a Community Development Co-ordinator and has also taken on the Coordination of the North East Lincs. Wheels to Work Scheme.

Carole White - Community Link Worker

Carole White - Community Link Worker

Carole is the new Community Link Worker, to promote and provide the community link of the Social Prescribing service for the Goole and Howdenshire area (shared with Carole Johnson).

The service will take referrals (for those aged 18 & over) from local GP practices, who for whatever reason may benefit from social and community based support.

There is no defined 'reason', but it could include people with anxiety, lack of confidence, low self esteem, isolation or those coping with bereavement, unemployment, carer responsibilities, pressures of parenthood or a change in circumstances.

Carole will offer a confidential and person-centred approach, including time to listen, to support and offer choices. It takes a holistic approach, focusing on the individual and considering the social rather than medical aspects of 'health and wellbeing'.

The ultimate aim is to enable each person to consider and explore other ways of improving their life and wellbeing.

Referrals via the GP can be made via the email: community.link@nhs.net (where they will be allocated to the appropriate Community Link Worker)
